If a spell costs 2 Sanity for a bonus to combat (e.g. Dread Curse of Azathoth), and the investigator has exactly 2 Sanity, does the investigator stay sane long enough to conduct the actual combat with the bonus, or is he devoured immediately upon casting the spell?

There are those who look at things the way they are and ask why . . . I dream of things that never were and ask why not

No. The investigator is driven insane, and in this case devoured, before the spell takes effect.

Thanks, Brian, thought that was the case. Some discussion over this in the rules:

"If an investigator goes insane (or is knocked unconscious or is devoured) as a result of casting a spell, the effect of the spell still occurs."

Was fairly certain the combat itself was a separate action.

It's worth noting that most spells will finish even if the caster goes insane. Combat spells are different because the effect is a bonus in the upcoming combat, and an incapacitated Investigator doesn't make it to the fight.

Yeah. Technically the spell does get cast successfully, but the Investigator goes insane before he can fight, so it doesn't matter.

A game is a series of decisions; a good game is a series of interesting decisions

Yeah, this makes Dread Curse a pretty rough spell in the Final Battle, since you have to recast (and pay sanity) every round of the Final Battle.
