The excellent Esoteric Order of Gamers summary talks about Italians discarding a card for each lost unit, but I can't find it in the rules, where does this rule come from? Also, does it happen on losing any unit or just Italian units?

It comes from the Equipment pack.

Quote:
Italian High Command
When the Italian High Command rules are in effect its army starts the battle with 6 Command cards in hand. Each time a unit is lost, one Italian player’s Command card is selected at random by his opponent and discarded. However the number of Command cards held by the Italian player cannot be reduced below three.

Since the rest of the rules for the Italian High Command is verbatim, it appears there was an omission in the original Mediterranean Rules. Which makes sense because they talk about how bad they are but then give them 2 benefits and no negatives.

Clarification on when to use it is on DOW's compendium:

Italian Army Rules were introduced with the Mediterranean Theater expansion in 2008. Official scenarios before that using Italians do not use them.

Bullet Point #1 was added to the card with the Equipment Pack in 2012, instead of creating a "6b" card.

As a result, early scenarios with Italian Army rules (such as Raid on Barce) use only Bullet Points #2 and #3.

Similarly, scenarios with mixed forces (Italians & Germans) would not use Italian High Command rule, but would use #2 and #3.

At this time official scenarios designed for Italian High Command rule include:
Battle of Nezuet Ghirba (EP),
Yasnaya Polyana (EP bonus #4),
Cavalry Charge at Chebotarevsky (EP bonus #5),
Operation Little Saturn (EP bonus #6),
Italian Diversion at Gazala (EP bonus #8), and
Battle of Agordat (French Open 2013).

Variant (house rule):
If all your units on the board are Italian, I suggest you use Italian Army rules including Italian High Command rule and start with 6 cards.
Beda Fomm,
Oath of Kufra,
Gallabat & Metemma,
Raid on Barce.

This looks like a very reasonable approach. Will make use of the suggested house rule in future. Just a bit closer to historic realities.

And note this is the revised card with Equipment pack, not the original from MT.
