This happened in a multiplayer duel, but it could have happened in any duel:
First round of the game, someone plays their signature spell Immolate. It triggers at RN 6, which happened to be first. Player rolled a 6 for damage, immediately killing the Witchborn before their very first spell of the game could have triggered.
Anything I'm missing? Seems obviously problematic.

Nope, that works as intended. You're free to make house rules against signatures you don't like, or avoid using the witchborn/archetypes but, immolate getting a 6 is very rare and does have its counters.

Hush, Exalt, Barrier, Transmute, Evert, Resurrect, Specter, Equilibrium, are all good ones that would prevent a 6 DMG immolate from being too deadly.

I highly recommend Specter or Resurrect for the Witchborn as a guaranteed failsafe.
