1) If I want to be an allied defender - who am I supposed to ask for permission: the main defender or the leader of the city? (supposing of course the main defender got the leader's permission earlier)

2) If I want to be an allied attacker/defender - must all other attackers/deffenders agree?

It's been some time since I played this, so my memory is a little fuzzy here, but I believe the answers are:

1) Both/all of them. A city-state becoming an allied defender will become allied to the city, the main defender, and any other allied defenders. Note that it is the city-states involved, and not the players (in the case where one player controls more than one army). (Also, if a player controls more than one army they are automatically allies. Even though the player might wish otherwise.)

2) Yes. If you want to attack a city, you cannot be allied to the city you are attacking or any of the defenders. You will become allied to all of the current attackers if you are not already. Nations must mutually agree to ally with one another. Note that alliances are not transitive.

If army A attacks army B it is possible that both A and B are allied to C, however C cannot take part in the battle. On the other hand, if A is at war with B, A is allied to C, and C is already defending (alongside B) then A simply cannot attack B in that particular location (since it would mean attacking their ally C).

I believe you have to ask permission to become an allied attacker/defender in a particular battle even if you are already allied with the appropriate players.

The intention is that players cannot be forced into alliances (since they are binding for the duration of the ... war? ... I've forgotten the term used for set of battles in this game). You might want to comb through the forum for rules threads that MW involved himself in, not all of the rules needed for play are in the rulebook for this game.

I've read all Martin's replies but I still had these doubts. his answers simply didn't cover them.
The "war thing" is totally confusing but hopefully works well during play :)
