I think, in this case, the first player who got two of his tokens out safely wins.
By extension, when both players lose three characters, whoever got his single token out safely first wins.
(I assume that you mean permanently killed, when going through the monster-step deck a second time.)

You apply the same rules as in any other number of players. Even with ties in numbers, one player was the first to put those tokens out. So if one each, the first one to escape with his/her one token wins. If two, the first one to save the second token wins, etc!
