Hi I have a nephew and he is very smart, he spends a big paret of the day in the day care and in the afternoons his mom gives him a lot of attention but she also needs to do the house duties so I'm looking forward to provide them with some games they can enjoy together or that he could do on his own and give his mom a brake.
I also have a passion for design and since nice games are hard to find I am willing to make my nephew's games.
I only need ideas, he loves puzzles but he is too good at them and knows the ones he already has by heart. I would like something that develops his basic skills, specially reading skills cuz he is a bit slow in learning to read but he loves books.

If he likes puzzles, then he might enjoy The Kids of Carcassonne. The game still has tile laying so players will create a landscape. It requires color recognition and basic counting. These are two skills that should be doable for a four year old but not yet be beneath him.
He might also enjoy playing with the pieces on their own to basically make his own free form puzzles as he configures different landscapes with the tiles.

I have researched this question a good bit for my 3yo niece and 6yo nephew; also played a lot of games with them. Some of their favorites, and enjoyable for adults to play with them too:
Kids of Carcassonne: great tile-laying game Gulo Gulo: wonderful dexterity game. Kids are better at it than adults! Sleeping Queens: card game viva topo: race to get the mice safe, harder than it seems at first magic labyrinth: very cool planning/memory game plus symbol matching Chicken Cha-Cha-Cha: memory game, but with fun twists The Secret Door: cooperative memory game Catch the Match: clever, simple picture-matching game Tier auf Tier: Great dexterity/balancing game that is fun for adults too For working on reading skills, you might consider Scrabble Jr.. It gets mixed reviews here, but I enjoyed it as a kid, and my niece and nephew enjoy it too.
