Does that +2 go towards the card, or the card you play in defence?
If the latter, you would always win with a blue bean 20 +2 = 22

I assume you cannot attack a city without a canon?

What happens if the defender puts down a card

The +2 goes toward the card that you play for defense, and you always have to play a card even if you're planning on taking the 18. If that total is less than 18, then the city's defense still applies. You always win on defense with a blue bean anyway, as a cannon does not break a tie at 20.

You can attack a city without a cannon, but you will need a blue bean to have a chance of winning. If the defender has a blue bean, then he wins. If the defender has a fortress and a chili or blue bean, then he wins.
