I received some rule clarifications from John Goodenough (the designer).

Ritual tokens

Q: Do I draw and place the Ritual tokens one at a time?
A: Yes.

Q: When I draw a Skull token, but there is no match and the Ritual track is full (6 cards) and all slots are filled with a ritual token, where do I place the Skull token?
A: If the ritual track is full with 6 cards and there are still no ritual cards with a matching symbol, the token must be placed on the leftmost ritual card. Please see the section "Placing Ritual Tokens" in the rulebook on page 9 for more details.

Q: May there be more Ritual tokens than slots on a Ritual card?
A: When a ritual card has all of its slots filled with tokens, it's effect is resolved and then placed next to the stack of primal god cards until a primal god awakens. Once a primal god is awakened, completed ritual cards are simply discarded. Because the ritual cards constantly cycle through the ritual track in this manner, there should always be a ritual card available to place a token on it.


Set the Deceiver Skull Omen effect

Q: How many ritual tokens do I have to place on the Ritual cards, if there are more Ally cards on Set than Ritual cards in the Ritual track?
A: The ability for Set should be worded so that you only draw and place a ritual token for each Ally that was placed during the turn.
For example, if 2 Allies are placed on Set, you only draw and place 2 ritual tokens. Any Allies placed on Set during previous turns do not affect how many ritual tokens you place.


Miscellaneous

Q: Harridan the Stalker special ability: What happens on a die roll of 1?
A: There is no effect on a 1 result; that round of combat is resolved normally.

Q: Var Ni the Nightlord Omen effects: Do I place the discarded Ritual cards under the Ritual deck?
A: No, when the ritual cards are discarded they should be placed in a separate stack faceup. This effect makes the ritual decks run out faster, which forces the next highest rank of rituals to become the current ritual deck. Please see the bottom paragraph of the section "Drawing Ritual Cards" on page 10 for more details.

Q: When the Hero ends his movement on a space with the Lost City and an adventure counter, may I decide not to have a Market Step but to attempt the challenge?
A: Yes.
