If an Awful Green Thing is defeated in combat, is it permanantly destroyed from play, or does the counter go back into the reserve pile for future use?

It is returned to the reserve.

But the rules are quite different in each edition, so there is little continuity upon which to base clarity...

Actually the rules havent changed much at all. The wording is nearly unchanged from Dragon Magazine to my 5th ed to the newest version.

This may be the problem, actually, I think the term "Removed from Play" has taken on a more precise meaning (i.e., removed from the game entirely) since the first edition of the rules a couple of decades ago, when I think everyone understood they were just taken off the board.
