What happens if I possess a player and he has an Island? Do I get the card that is set aside?

No. But you can tick them off mightily if you bury their Possession forever on the Island

Here's the general rule: According to the card (and rulebook), you only gain cards which you caused the possessed player to gain.
"Gain" is a specific term used in the game, and buying is a type of gaining. Cards which are moved by any other term ("set aside") or any other type of object which the player gets (VP tokens) do NOT go to the possessing player.

The above answers are correct. Just to be precise though...
If you Possess a player and force him to play an Island and put it and another card on his island mat, they go on his island mat and stay there until the end of the game, at which point they are added back into his deck.

Or worse, their Platinum.
You probably shouldn't buy Islands when Possessions are in the game.

Islanding the Platinum can be bad for the Possession player too. Now, if all they were going to get out of it was a Dutchy, then it is probably OK to bury that platinum. Just know that if you get to possess them again you won't see it either.

My (base game) Dominion rulebook says: "In the Buy phase, the player can gain one card from the Supply by paying its cost."
You're saying that a better interpretation would be "buying and gaining are different, but buying causes a gain to happen". I had never considered that before, but in the technical world of Dominion, I can see it.

I think when this has come up before, it's been in the context of something like buying a Mint with a Royal Seal in play. Mint says that you trash all treasures in play when you buy it, and Royal Seal says that, while it's in play, you can top-deck any card you gain. Because buying happens an instant before gaining, the Royal Seal will be gone before its effect can be triggered.

I seem to remember that this also came up with a different, more obscure, situation, but I don't remember what it was.

Besides Island, other fun cards to have on the table with Possession include Masquerade and Ambassador.
