I haven't followed the forums that closely, but I wouldn't say the Premium part of the rules has given us much trouble, actually none so far.
I don't know about the cover, I couldn't care less of what's onm the cover. Main thing is the game is good, and it's not......it's great! 
That being said, I'm fairly sure there won't be a reprint :/

Just bought this game last week and played a 2-player game with my wife - we both enjoyed it.
There are still lots of copies available, both in the US, and also in Europe (we ordered the English edition from Amazon Germany), so I doubt a reprint is coming (or is needed).
The rules seemed very clear to us - I haven't browsed the forums here, but we didn't have any rules questions come up during the game that weren't covered.
I think the cover is fine - if anything, it is rather tame compared with the original Paolo Parente comic (which my wife bought for me, and again, she has no complaints about the illustrations in there).
Great game - I've been fancying a bit of conquest gaming lately, and this and Conquest of Nerath (which still has to hit the table) were the two games which I in the end decided to buy, as they both seemed to work okay on two players.

I'm not even kidding when I say that I hope this never gets reprinted, and the ONLY reason for that is for every last one of you that complained about the box cover. How much of a complete prude can one possibly be? It's not like they stuck a couple having sex in the middle of the Atlantic Ocean on the map.
Solution; Buy game. By 99 cent gift box or rubbermaid container at a dollar store. Put game in new box. Throw old box away. Of, if you don't want to throw the box away, get a marker and scribble over and censor whatever you consider objectionable. Or cover it with some duct tape. Or Disney stickers. Or USPS labels (free at every post office in the US) Crazy, I know, but utterly simple low or no cost solutions to your "problem" are abundant!
I can honestly say that if it weren't for BGG, I wouldn't even know what the cover to half of the games I've played in my life look like. If you're sitting around looking at a box cover, you're almost certainly not PLAYING YOUR GAME right. Just saying if you choose not to play or buy a game simply because of the box cover, you deserve to never own or play it.
But, this whole debate over this completely asinine subject did give me my all time #1 favorite phrase ever written on BGG "whore on the box cover".

Be nice to see new art work, box and map.
I really hope to see a real map board instead of the crappy puzzle they put all warped in the box. Complained to FFG and no reply, sent em photos, still no reply.

Yea it was hard to put together, even harder to keep it together. Tried leaving weights on it to flatten it out. Tried bending it. I have had birthday cards made of better cardboard.But on the good side FFG sent me a new board last week.....the last one they had.
What I am thinking of now is to glue the old one to plywood.

OK, mine was perfect. Only complaint with the board is that they made the pieces a bit longer on the one side, and they only fit in the box on one side (meaning two of the four ways it can be turned). And, to me atleast, it's not apperant, which side they have to be turned. But that's a small thing.
