If I build into a city milepost across a river that is contained entirely inside red artwork of the city (e.g. Hamburg), do I pay for both the river and the city?

Yes. The red marking has no influence. You travel (and pay) from one post to another post, so if you happen to cross a river, you pay the extra.

Hmm, I was gonna contradict Ken's answer till I took a look at the map. But yeah, he's right. I was thinking of the large cities where the river runs through them. For those, you can build into one side of the city and out the other side without building across the river. But for something like Hamburg, where you're building across the river, yep, you pay for it. Unless you can get the city council to pay for it by taxing the local residents. That usually only works for sports franchises, though.
