"Move any terrorist unit as far as you want on the board. You must either get agreement from each Empire as you use their 'air space' or pay 20m for each unauthorised country you pass through"

I assume the following for this card: You pay nothing, if you cross completely empty countries or countries with only terrorists (even if there is a terrorist player). If you have to pay, you have to pay per country and the money goes to... where? The player who didn't give permission? Or to the world bank? Both seems thematically possible: either an "official fee" or a bribe of "lower authorities".

An additional question: I want to reach country D by a terrorist unit with extraordinary rendition. For that I have to cross a chain of other countries which belong to the same empire than country D itself. The player of this empire presumably wont give permission for such an action. So, can I pay this player 20m x number of crossed countries to place the unit in its own area?

Yes! You don't need permission to "land" as it were, just traverse through the air space. If the air space isn't owned, there's no one to pay or seek permission from.
