How do you deal with the rats if the Rat King is one of the starting cards? Lets say the Rat King is the second card from the top of the deck. Do you put the rats on the empty spaces and THEN fill the spot when the rats are killed or do you finish laying out the other four cards before covering them with rats? (I suppose a third option is to lay down the rats and then cover the last four with portal cards, but that just seems wrong.)

I know this has been asked and answered before. So look for that if no one official comes here!
They said that the rat king has had the text changed to be a fate card.
In the iOS game, all cards are laid out before their fate abilities happen.
I would infer that cards all get laid out and then covered up.

I agree. Populate center row as usual. If one of those starting cards is the Rat King, then simply cover the others with Giant Rats. Problem solved.
