The Virulent Strain expansion keeps the tension there in the game the whole way through. Last game we played we died on our way to a research station to cure the last disease. Really exciting. I would so get this expansion.

I have played the Bio-terrorist variant and can say that I am not all that keen on it.
That said, the expansion as a whole is totally worth getting and is one of the best, must-have expansions for a game out there. Go get it!

Seriously, this is one of my all-time favorite expansions. It ranks right up there with the Kingsburg expansion. I never play Pandemic without some elements from On the Brink.

I was looking for a co-op game to play with my wife and got Pandemic around Thanksgiving based upon BGG recommendations. She liked it so much that we got On the Brink for XMas. We almost exclusively play OtB now when we play pandemic. So far we have only played the virulent strain and mutation options (not the bioterrorist option) and it definitely makes the game more challenging without the obnoxiousness of an outbreak every third turn.
We also like the added roles. About half are blah, but the others make nice twists on the strategy. If you are in the 20+ plays of Pandemic and it's getting a little old, OtB will breath new life into the game. We consider it money well spent.

It is worth getting even if you don't use all aspects of the expansion right away.
The biggest fastest value add will be the additional role cards that do not make the game harder but do add more variety and change how hard a game can be.
Later you might tinker with other aspects of the expansion, but it is worth it for the additional Role cards alone (and the petri dishes)
