I'm fairly new to AH and a few nights ago my gaming group went up against the Ancient One in direct combat.
We quickly realized we were unsure about how to handle certain spell effects like "Shrivelling", "Dread Curse of Azathoth" and "Wither".
As we saw it, we could handle it in one of two ways:
1. Cast the spell each time we were to perform a combat check
or
2. Cast the spell ONCE and consider the spell as active throughout the combat with the Ancient One
In the end we went with the 2nd option since we saw the Ancient One combat as one combat scene. It did feel a bit wrong and overpowered, but it also was the choice that felt logical to us.
Which option is the correct one?
I'm sorry if this is allready explained in the rules. In that case, let me know where in the rulebook.

I don't have the rules with me, but you played it correctly. I think the rule relates to the fact that the spells are exhausted when cast and must be refreshed during the next Upkeep. In Final Battle, after the AO attacks, there is an Upkeep phase, at which time the spells are refreshed and must be recast.

Ah yes, makes sense, but we were wrong in that case. You see, we only cast the spell once during the entire combat with the ancient one. We never recast the spell after our upkeep. We considered it allready active.
So once we had successfully cast the spell once, we never cast it again.

Brian is correct. Ech new round with the AO is considered a separate combat for spells and equipment (such as the .357 Magnum). You get an upkeep between each round, where you can cast spells such as Heal and refresh those one shot weapons. When you make your attacks, you will have to decide then whether to cast again; if using epic combat cards, there may be factors to consider such as additional sanity or stamina loss or other mitigating effects.
