This unique ability is in effect during all players' turns and does not cost any action to perform.

I don't understand how this works out in play.

Is that last sentence just if a disease has been cured earlier in the game?

Bobby goes to a city with three black cubes. He chooses treat disease. Sue says, "I'll take all three." He used an action. Hers was special and free, even if it were her turn. I'm guess he did this because he knew that sue would take the three cubes for their strategy.

Also, does that mean Sue might have five possible actions?

[no I haven't played yet, just reading the rules.]

The "unique ability" is the Medic's ability to automatically remove all cubes of a cured color from any city, when the Medic's pawn is in that city. That is the ability which is always active.

Here are examples, from easy to tricky. Imagine that black has been cured and Istanbul has 2 black cubes.

1. It is Sue's turn (Sue is the Medic). Sue moves in to Istanbul, which has 2 black cubes. Both cubes are automatically removed without cost of an action.

2. Ralph, the dispatcher, moves the Medic into Istanbul (on the Dispatcher's turn). Both cubes are automatically removed. This is because the Medic ended up in the city.

3. Bobby is playing the Infector phase of his turn, and Sue is already in Istanbul. Bobby draws the Istanbul card. The black cube appears briefly in the city, and then is immediately removed again because the Medic is in that city.

The Medic cannot grant their power to another player at random (Bobby, in your example). If Bobby moves into a city, the Medic player may not say "Bobby, you can remove cubes for free now."

The point is that cubes of a cured disease can never exist where the Medic is. So, if any cubes of a cured disease are placed on a city with a Medic, they are immediately removed, no matter whose turn it is. Additionally, if the Dispatcher moves the Medic, cubes of cured diseases on the cities that the Medic is moved to also disappear.

All other players still have to spend an action to removed cubes of a cured disease where they are, though they get to do what the Medic normally does and remove all cubes of one colour with one action, instead of only one cube.

Yes, this is only relevant if the disease is cured - other people moving the Medic around can wipe out cubes.

The rest of your post was a bit baffling...

Think of the Medic as being "radioactive" and killing all examples of any cured disease anytime they share space.
