Look/Draw-

1)Is it correct that since all Action Card "Draw(s)" actions are taken at the start of the turn, and not at the start of the phase, the "extra draw(s)" do not require the play of a Look card? In fact, the extra draws happens immediately after the (default) draw of three Action cards?

Then, later, if a look card played to resolve phases has a named soldier then that named soldier can use his dog tag at that time to draw action cards again (if he has that ability under look on his soldier card.)

2) Hide/Hold-

Hold can only be chosen during phase resolution when a hide card is played for that phase? (Named soldier can additionally apply dog tag to hide cards, again, if applicable?)

1. Looks like you have it right. At the start of each turn, you draw the 3 default cards plus any extra draw you can get from alive/not captured soldiers. So if you have 3 soldiers with draws of 3, 2 and 1 respectively, then after drawing the initial 3, you would draw an additional 6 from the total draw values of your leader soldiers with draw values.

Yes, later if you play a Look action card and the named soldier on that card has a Draw value on his card, then you can use that dog Dog Tag action to draw that many more cards. Helpful for initiative switch.

2. Hold can only be played during a phase where the Hide action is allowed. If you play a Hide card during a Hide phase, the named soldier on that Hide card has to have a Hold value on his card to be able to use a Dog Tag action to Hold cards. Some soldiers with Hold values on their card don't have a named Hide card as one of their action cards, so they can't use the Hold action. Though I believe there will be circumstances such as some Team cards on Tactics modules that may allow for a Hold action to be played, but not sure about that. Jeff did mention that there will be other ways at some point to be able to use a Hold action for a soldier without that soldier having a named Hide card.

Bingo! Good explanation of an area where folks seem to get a bit muddled.
