The card tells you to increase dmg whenever you attack the target with the fewest HP, any other target you subtract one dmg.

If there is only one non-hero target, it has the lowest HP but also the highest, so does the effect cancel out? or do you count it as the lowest and boost dmg?

What if two cards tie for lowest, do you boost dmg against both or pick one for boost and one for less dmg?

The way it's orded it checks is this target the lowest. If yes, damage boost, if not damage reduction. As a single villain target would be both highest and lowest it checks the box and gets a damage boost against it.

As damage is not simultaneous in Sentinels, the first one would be the lowest and would get his for the bonus. The second would no longer be the lowest as the first one got hit and, presumably lowered its HP, so it is now lower.
Of course, if the HP weren't lowered for some reason, the second would still also be the lowest and increase damage dealt against it.

It also has the highest, but interpret the text literally. Damage to the lowest HP target is boosted by 1. Damage to any other target is decreased by 1. That single target is not an other target than itself, so damage to it is +1.

In cases of a tie, the players choose, and that choice can change. Suppose there are two minions, A and B, both at 4 HP (and they are the lowest non-hero targets). You use Taiaha, hitting A (choose this to be the lowest target) for 3+1, killing it. For the second damage of Taiaha, you hit minion B, which is now the lowest target, for 3+1, killing it.

However, if they were both at 5 HP, you would knock A down to 1 and would then get a damage reduction against B, because A is still the lowest.
