Are the expansions playable with end game vanilla characters?

This is the end; hold your breath and count to ten; feel the earth move and then; hear my heart burst again

Not sure what you mean by "end game vanilla characters".

All the heroes work with all of the box sets. You can use expansion heroes with the original quests or vice versa.

Also, to be clear: the heroes start fresh for the expansions, they don't keep their experience.

Enjoy!

each campaign is a stand alone... everytime... it would not be balanced at all to let the heroes keep the exp for and do another 9 quests... it would just be an endless steamroll that the overlord would have to endure for 9 nights being able to do nothing but sigh XD

but ofc u can use the 8 original champions in any campaign starting with 0 gold/0xp (same for the overlord ofc)

There is no continuation in between expansions.
Big expansions (Labyrinth of Ruin and Shadow of Nerekhall) offer new campaigns like the "Shadow Rune", starting anew with whichever heroes your team likes.
Small expansions (Lair of the Wyrm, The Trollfens) have a set of quests that can be incorporated in any campaign you play. Also, their quests can be played as mini-campaigns.

There has been some talk of a fan-made Act III variant, but I don't think a good conversion has come up yet.

so is it "safe" to skip the small expansions then and just go for the big expansions after the core set?

The small expansions are good choice to be bought, because they work with the big expansions/core. When you play the big expansion, you put aside other big expansions (maybe you can use monsters, but not the quests).
