Let's work a little bit together here:

What if there was a deck of cards, with each card granting a Unique Power. Before the game begins, each Cultist draws one card and places it before him. Then start playing.
The gameplay should remain as simple as it is, so no introducing new Dice or Tokens, the idea is not to have a balanced game, but to introduce a little bit more Chaos and Fun.

Here's a couple of ideas (Flavour-text and Thematic Headlines can be added later):

- You start with one extra Sanity Token (if you steal this card during the game, you immediately get 1 Token from the reserve - not from Cthulhu).
- If you roll an Elder Sign (not choose!), you may take 2 Tokens from the middle, instead of 1.
- If you roll (not choose!) Cthulhu, you may choose one player, who loses 1 extra Sanity Token
- If another Player rolls (not chooses!) Cthulhu, you may take his Power and give him this card instead. When you roll (not choose!) Cthulhu yourself, you may choose any Player to switch Powers with (at the end of your turn).
- If you didn't roll the Eye-Symbol, you may immediately roll a second time. Your second roll counts.
- If you didn't roll (not choose!) the Elder Sign-Symbol, you may immediately roll a second time. Your second roll counts.
- If you didn't roll (not choose!) a Tentacle, you may immediately roll a second time. Your second roll counts.
- If you didn't roll (not choose!) a Curse-Symbol, you may immediately roll a second time. Your second roll counts.
- If you roll (or choose) a Tentacle as a Reaction-Roll, you only lose 1 Token to the Attacker, if you're among players with the highest Sanity.
- When you roll (not choose!) Cthulhu as a Reaction-Roll, you gain 1 Sanity Token from Cthulhu instead of losing 1.
- If another player rolls (not chooses!) Cthulhu, you may immediately defend by rolling the die yourself. If you roll (or choose) Cthulhu, only the active player loses 1 Sanity Token to you.
- If you roll (or choose) a Tentacle as a Reaction-Roll, you may hand the lost Sanity-Token to any other player (or even Cthulhu himself), instead to the Attacker.
- Right before the game starts, you may choose one card from any Player at the table, that you want. That Player then draws a new card.

Here's some more (I'm actually considering to design those cards - to make this a custom expansion )

- Before the game starts (or when you've just stolen this card by any means), place this card between you and the player either to your right or your left. With this, you copy their Cultist-Power.
- Whenever someone rolls (or chooses) Tentacle against you (also as a Reaction-Roll), they also gain this card, and you'll get theirs.
- After your turn, before you pass the die to your left-hand player, roll it once more: if you've rolled Eye, Cthulhu or Elder Sign, you may not be attacked, until again it's your turn. Turn this card to the side to mark that you're immune.
- If you roll (not choose!) Cthulhu, everybody else must turn their cards over (they lose their power). Then discard this card and draw a new one. The other Playersr may only regain their power, if they roll (not choose!) Elder Sign.
- If another Player claims the win, all remaining Sanity goes to Cthulhu, then roll the die. If you rolled Eye, then everybody gets 1 Token and you get 2 Tokens. Continue playing. If you rolled Cthulhu, everybody gets 1 Token and you get nothing. Continue playing. If you rolled Elder Sign, you win instead. If you rolled any other symbol, the winner stays.
- If on any players turn Cthulhu wins, ignore the roll and it's result, discard ALL Power-cards instead, gain 1 Sanity-Token (only you), and continue playing.

I took Other glass beads and assigned them powers:
Red: When another player rolls the tentacle the sanity goes to this player.
Black: The elder sign gives this player 2 sanity instead of one.
Clear: When this player Rolls the eye they gain 3 sanity.
Yellow: When this player rolls the yellow sign it counts twice.
Blue: can choose to reroll once during turn.
Brown: dunno haven't came up with this one yet.
