The Leap attack card states that "If an opponents health is lower than yours..."

Does this mean Base starting health or current health?

Also the "Leap" card says that if the opponents health is lower than yours you may use your Defensive rating for the attack...

Does this mean that if my health is lower than my opponents, then I can't use the card at all, or am I allowed to use the card but roll my regular Attack dice?

The leap card is an attack card. It can be used to make an attack at any time that is legal. If your CURRENT health is more than your targets CURRENT health, you get to use your Defense value to attack. If not, you attack as normal.
