Is the terrain effects accumulative? So, a unit in Brihuega has a defense x4 (Dx2 by Village and Dx2 by Hills)

Brihuega is in hex 18.03. There are no Hills in that hex. It would be Dx2 for the Village.

Brihuega is light brown, no white. Brihuega is hill and village.

White and light brown are both Open terrain. If you look closely at the image example on the Terrain Effects Chart just to the left of "Open" you can see that it shows both white and light brown. Only the darker browns are steep enough to be considered "Hills" terrain.

What about 16.05, which is hill, woods and Palacio, each of those terrains have a Dx2 effect.
Are they cumulative (which would mean a Dx8 effect) or is the defense only Dx2?

From Map Notes-- Page 2

The defensive benefit of Palacios is not
cumulative with the defensive benefit of other
terrain in the hex (i.e. the defensive benefit for
these hexes is 'D x2').
