Played this for the first time yesterday and something caught me as being odd. With 23 turn spots per round, and 4 rounds in the game, the last player gets shorted 1 turn at games end. Did we play something wrong or is this by design?

Nope, you played it wrong, it's 23 each.

Because it is very easy in this game to lose a turn my advice is to mark the next field where the first player is going to lay down his rondel tile with a spare game bit (say a coin). This provides immediate feedback in case the first player ends up on a different field.
