Can anyone explain how the expansions tie into the base game?

Do they just extend the map and have a different starting point?

I believe the starting point is always the same. Expansions add areas to the map and the corresponding curse leads you to that area at some point.

The curses of the three extra curses are already foreseen in the base game.

For each curse, there is a banner mechanism. When you need to draw a card related to a curse, there's a symbol corresponding to the curse besides the card number you're supposed to take. If you're playing the corresponding curse, you know you have to add a precise number to that target and you'll draw a different card.

The base game already contains the symbols for the add-ons.
