I'll use Sarafina's Hollow as an example:
"On Hit, Range 3: Gain +2 Power"

So On Hit means if it hits, it happens, but what does Rang 3 mean? That it has to hit the opponent at exactly 3 spaces away?

Exactly. When you hit them and are at range three you gain +2 power. Otherwise the full On-Hit clause isn't met and you don't get the bonus.

One thing to note - if you pair this with Grasp, which has On Hit: Move the Opponent 1 space, you can trigger that On Hit first and move the opponent so they are at Range 3, thus getting the bonus (it's a bit slow for a Grasp though - so not the best attack pair).

Very true. Triggered effects resolve in what ever order you want (Start of Beat, End of Beat, On Damage, etc) so if they are at range two and you pair it with Grasp, you can throw them to range three and then apply the Oh Hit damage boost. The reverse is also true, if they are already at range three you can trigger Hollow's On Hit for the extra power, then throw them to range four or haul them in to range two.
