I found this FAQ about the Therapist role on the wiki:

Q: If you use the Therapist's ability on a character with 0 paranoia, do you still have to notify the protagonists?
A: No, there's no need to tell them. Using this method to conceal the Therapist's existence is an important rule.

Are characters with 0 paranoia always a valid target for the Therapist, even when other characters in the location have paranoia? That would certainly make it a lot easier to keep them a secret...

Yes.

This ability has only 2 restrictions :
1) It can only target a character other than the Therapist himself.
2) The Extra Gauge needs to be at 1 or above.

So, the characters with 0 paranoia are always valid.

The only time you have to tell that something nearly happened is when an Incident is triggered (The culprit has enough Paranoia on the right day) while no effect has occurred.
