A quote from Brett Christensen's PDF on early AH D-Day 1st ed. here on BGG:

"It appears that “exchange at attack” is implied in 1961A. Both sides use attack factor for their exchange “combat factor” calculation."

D-Day 1961A:
"EXCHANGE: An even elimination of combat factors, the defender’s factor computed at original, double, or triple value according to terrain. When an exchange is rolled, the player with FEWER combat factors removes all his units – other player removes a number of Units removed buy the opponent. Since this exchange of combat factors does not always work out even-up, a player sometimes must remove a number of Units whose total combat factor is MORE than that of the Units removed by opponent."
