When a character moves into an unexplored space with no tile there, he then places a random new tile there with the arrow at the entrance. With the new tile in place, the character is actually just crossing right over to the new tile the arrow symbol at its entrance. So then the player ALWAYS draws a dungeon card?
It depends on which tile you enter. Some tiles you don't draw cards for, some you do. Consult the tile chart to see which. But assuming it is a tile you do draw for, yes, you draw it immediately upon entering the tile.
For example, you don't draw cards in hallways or "dark" rooms.
And when a character moves to an already explored dungeon tile, he always draws a dungeon card there as well?
Again, it depends on the type of tile, but if it's one you draw cards for, then yes, you draw a new one every time you enter.

Depends on the tile.
When does one draw dungeon cards, exactly? I am confused.
If you DO draw a dungeon card for that tile, you draw it immediately upon entering the tile. If you STAY in a room (because you tried to lift a portcullis or are searching), you do not draw a dungeon card (but for searches, you would draw a search card).
