We have been playing Mare Nostrum with three players, but have lately wanted to try playing on a larger map. Do you think barbarians could take the place of a 4th player on a 4-player map?

I have no idea, but Mare Nostril made me snort.

That actually sounds like an intriguing idea to me. Some Mercs for hire.

There is some historical precedent for this:

We have been thinking about doing this as well. We played a 3 player game on the big map (base + atlas expansion) and up the cost of h/w cards by 2 with no pyramid win option. This was pretty fun as we had atlas, Egyptians, and Babylonia. it would have been nice to have some barbarians in the roman/greek area.
