I just got this game after seeing it played on Tabletop. My group loves One Night Ultimate Werewolf, and this seemed to be a nice alternative for something more of a campaign. As I'm looking over the score tableaus, I notice that when it reaches 7+ players, it says "2 fails required" above round 4. What is this supposed to mean? Are you supposed to have 2 failures before even entering round 4? I can't find anything in the rules to support this at all.

Missions marked with an asterisk (*) require two fail cards to be played in order for the mission to fail.

For mission 4 (only), when playing with 7+ players, that the mission will be successful unless two different FAIL cards are present, meaning two or more spies were on the mission and at least two of them played a FAIL.

Just to be abundantly clear the 2 fail requirement only exists on certain 4th missions (7 or more players), never the 5th mission as you seem to indicate.
