In the manual there's a sentence:
"if the energy demand space that matches the current Decade number is empty or is occupied
by a green power plant. If it has a green power plant, or all spaces in a region are totally flled, move on to the next region."

This is what I understand, according to the description of manuel: On decade n, We check n-th demand space, if it's empty, we place a fossil fuel power plant, add up the CO2 pollution level.

If my understanding is correct, why we care if "all spaces in a region are totally filled"? Shouldn't we just check n-th space during decade n?

That is correct.

If the nth space is empty it will need a fossil fuel plant and the CO2 level increases.

I think they added the other text to make sure the players did not replace the last power plant or to reiterate that you can skip that region.

For example, Africa only has 3 spaces so their energy requirements are completed in the 1990s (3rd decade) after that they no longer need anymore power plants to be built.

You can however propose and install in those regions even if all 3 spaces have clean power plants.

Thematically, it doesn't make sense to allow players to propose or install new power plants if all spaces already have green power plants.

It does when you factor in that the governments are corrupt enough to accept those proposals. It happens in real life where budgets for some projects get greenlit and then do not get completed.

It says so in the rule book itself.
2.1.3 Power Plant Construction, Page 6

Note: Green Power Plants cannot be replaced. If a region only has green power plants, it is no longer possible to build power plants in that region. Any leftover installed projects in the region remain on the board. You can still propose and install projects that the region no longer needs. The regional government is corrupt enough to pay you anyway.
