Psychiatrist
Sane: You have 5 Sanity. Once per turn, as an action, another sane investigator in your location regains 1 Sanity token.
Insane: For 3 actions, fully regain your Sanity if there's a sane investigator in your location.

Underground Worker
Sane: You may do the "Take a Bus" action regardless of where you are. You can't Seal Gates.
Insane: For 2 actions, move to anywhere on the board.

Antiques Dealer
Sane: Whenever you Defeat 3 Cultists in the same turn, gain an unused Relic.
Insane: Whenever you Defeat 2 Cultists in the same turn, gain a random Relic that has been returned to the box.

Cult Infiltrator
Sane: Whenever a Summoning Ritual occurs, take 1 Clue from the discard pile.
Insane: Whenever an Evil Stirs is drawn, take 1 Clue from the discard pile.

Psychiatrist seems very good. The game has very few mechanics for recovering sanity.

My ideas:

Scientist
Sane: Can Give and Take Clue Card even if the card does't match the town it is in. Card can be given to the Scientist or can be taken from it only if it matches the town where it is.
Insane:Can Give and Take Clue Card even if the card does't match the town it is in, but it costs 2 actions. Normal Give or Take card costs 1 action. Card can be given to the Scientist or can be taken from it only if it matches the town where it is.

Priest:
Sane:At the place, where the priest is, no cultist or Shoggoth can appear.
Insane:Sane:At the place, where the priest is, no cultist or Shoggoth can appear. Can defeat two cultist for one action.

Librarian:
Sane: If active, the Librarian can check the next, still inactive Great Old One card in advance. If a Librarian is in play, at the beginning of the game shuffle +1 extra relic card to the Player Cards and remove a random Clue Card without knowing its color during the game. The relics don't count as a part of the Librarian hand.
Insane: If active it can look at the next, still inactive Great Old One card in advance. If goes insane, Librarian plays every relic from his hand immediately in its next turn.
