This seems obvious at first glance, but in order to be able to climb to the highest Trophic Level within a domain (marine or terrestial), there have to be at least two other macroorganisms within the same domain, right? If your macroorganism is the only one, then you are a plant, so to speak?

Yes. Exactly what you said. You start as a plant. When another macro shows up on Earth you adjust trophic level accordingly to your organisms' metabolism (so you may go above new one or stay where you are and get eaten by new macro).
