Last page. Check out second edition rules if you haven't. Phases order, you mean? because there is no turn order.

Edit: that would take a bunch of cards, but maybe plausible.

ER is done differently, but everything else should work.

Yes, the main difference should be the game end, which is now triggered during Ambulance phase
