So from doing some research it seems this doesn't scale at all?
It seems this is a 4 player game and with less player you just play as if it is a 4 player game, correct?

You are correct. Unfortunately, due to the nature of needing to fulfill orders for each players' role, all 4 roles must be played with.

Solo Play – One player controls all 4 roles and completes all actions for each of them every round. Play will always begin with the Nurse role, but in Solo Play, the player can change the turn sequence of the remaining roles whenever necessary.

2 Players – Divide the 4 player roles evenly by giving 2 roles to Player One and 2 roles to Player Two. The player with the Nurse role will control the first and fourth turn positions (starting and ending every round) and the other player will control the roles in the second and third position of the turn sequence.

3 Players – Divide the 4 player roles by giving one player 2 roles (the Nurse and any other role) and the other remaining players one role each. The player with the Nurse role will control the first and fourth turn positions (starting and ending every round) and the other two players will control the roles in the second and third position of the turn sequence, respectively.

4 Players – Evenly divide the 4 player roles by giving one to each player. The player with the Nurse role will begin every round and play will continue clockwise around the table until all players have taken all actions.

Unfortunately for me that's a deal breaker, we're not fans of playing multiple roles in game. And as an almost exclusive 2 player gamer couple this just isn't for us.

Definitely not a deal breaker for me. After working at the Army Medical Center/School, I am definitely interested in the theme, especially since you refuse the streamline the solo play because it doesn't make thematic sense. THAT'S a decision I can respect.
