When the rule book states to discard Triage and Tactical Decision at the end of the round, it is only to indicate when you would use them. (Just making sure you're not under the impression that you lose them even if you didn't use them.) Obviously, using them immediately after the die roll would not be relevant.

Once the emergency dice are rolled, the relevant cards are immediately activated. You could use them instead, provided it's not duplicating an effect, like extra dice that you may have already earned from completing a patient tile, since each player can at max have one extra die in a round (even though in 3-5 player games there are 2 dice more than what you'd use in a normal round).

In talking to Kane, I should add that there are some cards that may not be BEST to use in that way. Triage cards like A Fresh Start and Concentrated Effort are much better saved for when you find yourself stuck mid-minute. It may be worth re-earning one of them instead of burning it before the minute starts, when you don't yet know if you'll need them.
