The second scoring card was still somewhere in the money deck when we had less than a dozen tiles left. It didn't turn out to be the case (and maybe it's impossible), but we started wondering:

What should we do if we run out of tiles and trigger the "third" scoring round (final scoring) before the "second" scoring round happens?

You simply follow the rules as written for ending the game when tiles run out. It would be uncommon to miss the scoring triggered by the marker in the deck, but there is no guarantee for it to take place.

One sentence from the rules:

The second scoring ("B") takes place after scoring card "B" has been revealed.

There is no other provision for scoring B stated. So if the card does not appear then the scoring does not take place.
