I've seen this asked in a couple of places, and answered differently in each.

I'm wondering if there is a confirmed answer to this...

With Reality Checks, after checking for the colour, then drawing, for example, say, an Exploration card, to check for the appropriate script number...

...What do you do with that card?

Is it, A. Reshuffle it back into the Exploration deck,
or B. Place it in the Unknown deck discard pile?

Reshuffle seems the most likely to me.

I would think A, as once the unknowns deck is formed it stays that way (unless added to specifically by another event). So, a card drawn from the exploration deck should stay with it (and therefore be shuffled back in).

Either way I don't think it would matter much. The card that you drew was random, and whether its removed from that deck or shuffled back in it would still be random. I was trying to find a statement from AR about the game where it was mentioned something like 'when in doubt go with whatever feels thematically correct' for a basic rule regarding any question on what to do in situations like this.
