I'll add my vote for both shower and driving. But probably the very best place for me is during boring meetings at work. (Basically, anyplace where you can just let your mind wander....)

Since I work 5 nights a week from 11.00 PM to 7.00 AM at the front desk of a hotel, I get some awesome ideas at 3.00 AM. When it's just me and my thoughts and the internet.

I think a turbo-powered, diamond sharp kitchen sink would result in your creative juices [along with all other body parts] splattered on the surfaces of said kitchen.

Showering is number one for me, driving is number 2. My third would be sitting in a crowded room listening to people talk, no idea why, but it gives me good ideas.

My first two game design ideas since I really got into it seriously came while cycling home in the rain. Which does not mean I am looking forward to cycling home in the rain every day now.
Water seems to have an effect indeed. While showering hasn't yielded anything yet, I designed two short stories while swimming in a nearby lake, one of which was published.
