I just bought Pandemic and On the Brink yesterday and was wondering what diseases you guys use when playing. I was thinking of using the following: Red - Small Pox, Black - Black Plague, Yellow - Yellow Fever, Blue - Anthrax, and Purple - the fictional "Reaper" Virus ('cause it just sounds cool). How about you? What do you save the world from?

blue-blubonic plague (!)
black-the black death(!!)
red-the RedDeath...sometimes scarlet fever..sometimes hemorrhagic fever
yes they are silly and blue is purposefully mispronounced and black is the same as blue

I haven't played yet, but when I was watching the video review, I was thinking of them as being:
black - black death (which is thought to be a combination of bubonic and pneumonic plague)
Yellow: Yellow Fever
edited to add: I have since thought that when I buy the game, I will make blue the fictional flu called 'Captain Trips' (from 'The Stand'). I haven't decided for sure, though.

and we haven't used the fifth colour from the expansion yet, but when we do I'm anticipating calling it "Purple"
Great topic for immersive purposes, and the contest thread was a good read.
The WHO keeps a list of diseases that they cover with their Global Alert and Response (GAR) program. In other words, these are diseases that they would act in a manner consistent with Pandemic.
Since the diseases in the game behave in such a regional manner, you could try to choose accordingly. Personally I like:
.SARS for blue
.Dengue fever for yellow
.Yellow fever for red (I know... not yellow?)
.and Smallpox for black
