This came up in the longest game ever, where one player effective hoarded all the ore and used them to build a giant knight army, and effectively managed to keep anyone else from building cities for some time.

If the barbarians attack and there are no cities at all, does the player with the most active knights get a defender of Catan card?

Yes. However, you should all have had one city from your initial placement. The only time there will be no cities is if no one had any knights in the first barbarian attack and they were all destroyed.

Yes, the player with the most points is the Defender of Catan even if the barbarians had no cities to ransack.

Why would building an army of knights prevent someone from building up cities...?
