While Sauron was the "All Seeing Eye", it turns out that he was almost certainly nearsighted. Newly revealed orc documents reveal that he appeared to do a lot of squinting. Considering he couldn't spot the 2 hobbits right in Mordor, this theory may be the best explanation of what happened. Backers of the "no depth perception" theory reject this claim as being based on flimsy evidence and any squinting he may have done was just for dramatic effect.

I prefer to think of it as him having tunnel-vision It's like he's looking for his car keys; he looks everywhere in the house and doesn't realize they're in his pocket until he's late for work...or in his case, dead.
