Can you play this card on someone else's turn? Or must you play it before their turn, indicate whom you're targeting, then rearrange before they draw?

I believe that you can only play action cards on your own turn.

You play action cards on your turn, so when you play "Confusion in Line" you identify a targeted player.

When it is that players turn, they will (if they wish) play their action card. Just prior to collecting their noble, you randomly rearrange the line.

And please note the randomly. You just pick up the cards, shuffle them and redeal. You do not get to arrange the line as you wish.

Thanks, that's what I thought. Then again, I was the target of the attempted out-of-turn usage!

You can choose yourself if you're a player.

If you're not playing the game, you cannot choose yourself.

That means that if you're just a bystander, watching, you cannot grab the card, target yourself and expect to be taken seriously.
