I would point out that one or two of these issues (Such as branded as opposed to generic medicine) are also prevalent in socialised healthcare systems. In effect the whole medical industry is a bit of a con, it's just that with private healthcare the burden falls on you, and in a socialised system it's picked up by everyone.
Which is why not only you see government insurers dealing directly with the manufacturers to get very low prices when compared to American prices, but you see reports on which doctors prescribe a brand name when the generic is available.
